Licences: Standing Permission - Release Forms

This is the set of forms to be filled out and signed by anyone who wants to give a standing permission for recordings of their pieces of work to be made available for others to study and learn from, that is, who do not want to fill out a separate form at each workshop or session.

Below is a cover letter explaining the Standing Release Form followed by the complete text of the Standing Release Form.

Standing Permission Form

INTRODUCTION

If you regularly give Release Permission in the same way at LearningMethods workshops and wish to save yourself the trouble of filling out a form each workshop, you may give an on-going permission (Standing Permission) which will only need to be filled out once. This Standing Permission will stay in effect for each workshop or class you attend until you specifically revoke the permission. You can revoke a Standing Permission by filling out and signing the accompanying form or by writing a letter to express your wish to end this permission. Either format must be signed by you and sent to David Gorman at his current address (or you can hand it to him in person).

To use this form, first fill in your details. Then decide who will have permission to listen to your material. You can give permission for anyone interested in the LearningMethods work to have access to your material or you can restrict your permission only to LearningMethods Teachers or Apprentice-teachers who have signed the Professional Licence Agreement that they will guard the confidentiality of the material and not share it with anyone else. Finally, sign the form and send it to David Gorman at the address on the letterhead above (or hand it to him),
