This is the set of forms to be filled out and signed by anyone who is giving permission for a recording of their pieces of work to be made available for others to study and learn from.
Below is a cover letter explaining the Release Form followed by the complete text of the Release Form.

Information about the Release Form and the Licence Agreements
You have indicated that you are willing to allow your recorded pieces of work in a LearningMethods workshop or class to be made available for study by other people interested in the LearningMethods work. Since these pieces of work are about personal issues, it is important for the protection of all concerned that the rights and responsibilities are spelled out formally and clearly.
Thus you are being asked to complete and sign this formal Release Form which specifies what sort of
permission you are giving and which pieces of work this permission applies to.
Anyone who obtains the recordings for study must accept a Licence Agreement that legally binds them to not share
the material with anyone else, to respect the confidentiality of the material and the copyright.
There are two types of permission you can give:
1. Restricted—this restricts the availability of your recordings only to certificated LearningMethods Teachers and formal Apprentice-teachers who have already completed a signed and witnessed Licence Agreement. These people will use the recordings to further their training as teachers of the work and develop their professional skills.
2. Open—this type of permission allows for the availability of your recordings to anyone interested in the work, primarily to further their understanding of and get help with similar issues of their own. There is a similar Licence Agreement for this permission, both on the recording and as a printed document accompanying the recording, but people do not have to sign it. It is an implicit agreement in the sense that listening to any of the Recordings constitutes their acceptance of the terms and conditions of the Agreement.
Copies of these Licence Agreements are either included or can be obtained from the LearningMethods office (see letterhead above for phone numbers, postal and e-mail addresses) or from www.learningmethods.com/licences.htm.
On the Release Form you choose which type of permission you wish to give by putting a check mark in the appropriate tick-box (make sure you only check one box OR the other, NOT both):

The rest of the Release Form should be straightforward to fill out. There are two copies: one for your records and one for ours. Examples are below:

Only you can know if it is appropriate for you to allow others to learn from your experiences and which sort of permission is best! Please make sure you feel completely comfortable with your decision before signing.

INTRODUCTION
You are giving permission for your contribution(s) in a LearningMethods workshop or class to be listened to or watched by others. This agreement spells out what sort of permission you are giving and which pieces of work this permission applies to. This clarity is important for your protection as well as that of others.
First decide whether you are giving permission for all your contributions in the whole workshop/class or only some pieces of work. If you are giving/withholding permission for only some pieces of work, you need to specify which pieces below.
Then decide who will have permission to listen to your material. You can give permission for anyone interested in the LearningMethods work to have access to your material or you can restrict your permission only to LearningMethods Teachers or Apprentice-teachers who have signed the Professional Licence Agreement agreeing that they will guard the confidentiality of the material and not share it with anyone else. Conditions:
1. I assign to David Gorman (the “Teacher”) the rights to use my personal contribution in all media now known or which may be developed in the future provided this use shall be only in relation to the LearningMethods work or work developed directly from it. I reserve copyright and all other rights on my own contribution in relation to all other uses.
2. I acknowledge the copyright and all other rights of the Teacher in his contribution in this engagement, as well as the copyright and rights of all other contributors.
3. The Teacher shall only be liable for any loss or damage to me or my property if it is directly connected with this engagement and caused by the Teacher’s negligence.
4. My contributions or any future use I make of them will not infringe copyright or similar rights nor bring the Teacher into disrepute or be defamatory.
5. This agreement shall be interpreted in accordance with the Laws of The United States of America and I submit to the jurisdiction of the United States of America.
